Scheduling and Cancellations:

Please call our office at 408-293-1992 option 2 to schedule your appointment. You will receive a reminder call 24 hours prior to your appointment.

We kindly ask that you notify our office 24 hours in advance if you are cancelling your appointment. Appointments must be cancelled no less than 24 hours before; otherwise, a cancellation fee will be charged.

After Hours Care:

There is always a physician available after business hours to handle your urgent medical problems. You can reach the on-call answering service at 408-293-1992. We ask that you use this service only for non- life-threatening medical emergencies that cannot wait until the following business day. If there is a life-threatening emergency, please contact 911 or go directly to your nearest emergency room.

Medications

We ask that all patients bring his or her medications and supplements to the office visit for our nurses and physicians to review. This helps us keep an accurate record of all medications prescribed by all medical providers involved in your care.

Prescription Refills:

Please request all Cardiac medication refill prescriptions during your visits, if possible.

If it is safe for you to receive a refill without having a checkup, we will be happy to authorize your pharmacy to refill your prescription. If there are concerns, you doctor may ask that you come in for a visit before additional refills are approved.

IMPORTANT – DO NOT WAIT UNTIL YOU ARE OUT OF MEDICATION

Please allow 5 business days for processing. Your physician may be out of the office, in surgery or out of town and this may delay your refill.

Payment Policy

All co-payments are expected at the time of service. Deductible and co-insurance amounts not covered by their supplemental insurance plans are patient responsibility.  For your convenience, American Express, Visa, MasterCard, and Discover are accepted, in addition to cash, and checks as method of payment. If the co-pay is not paid at the time of service, you will be assessed a $10.00 billing fee.

Insurance

Our billing office transmits electronically to all insurance companies. Patients are expected to provide the office with complete billing information, including mailing addresses and certificate number.

It is the patient’s responsibility to know their insurance policy. Patients should be aware of their benefits coverage including which physicians are contracted with their plan, covered and non-covered benefits, authorization requirements, and cost share information such as deductibles, co-insurance, and co-pays. If you are not familiar with your plan coverage, we recommend you contact your carrier directly.

Your current insurance card is required at the time of each visit at check in. It must be on file for us to bill your insurance.

Our insurance contracts require us to collect co-pays and co-insurance at the time of service.
